I was contacted by a prospective client last week and asked why I always complete a new client wellbeing consultation as part of the first appointment and why I need this information?
What information do we require?
We need your contact details, your: name, phone number, address, postcode and email address.
We will also need information about your age and health in order for a treatment to go ahead.
We also require a signed consent form, if this is for a child under 16 we need parental consent too.
We need all the details above to match the correct treatment to the symptoms, ensure that it is safe for us to provide the treatments you may ask for and also for insurance purposes.
The questionnaire also provides a starting point to be able to re-assess the treatments are working effectively.
New data protection laws mean we need your permission to stay in touch. I will be emailing my Blossoms later this week with further details.
Our Blossom Promise to you;

We will only use your data enhance your experience with us and will keep you informed of birthday goodies, new treatments, products, freebies, workshops, courses, masterminds, retreats and surprises.
2. Do we share?
We will not share your information with anyone outside of Blossom Wellbeing Cheshire. We’ll protect your data like it’s our own.
3. Straight from Blossom's mouth
Do you ever find rules and regulations seem to be written in a foreign language? It is our promise to you we will explain everything in plain, simple English.
4. You always have a choice
You do not have to share your personal data with us, but if you do not, we cannot offer you a treatment with Blossom Wellbeing Cheshire. However, you decide how you hear from us.
5. We only keep what we need by law
Our promise to you if we don’t need it, we’ll delete it.
